We have an exciting new opportunity to join our Lexington Orthopedics team as a Care Navigation Coordinator! Position Summary: Under general supervision, this position coordinates referral requests and communication of appointments using general clerical and customer service skills including, but not limited to, organizing requests from multiple locations, obtaining preauthorization, and communicating status of appointments. This position acts as a patient educator, care coordinator, system navigator, and patient advocate with the mission to improve the care experience. This position coordinates pre and post-surgical care.  Provides pre-surgery education, coordination of medical clearances, post-surgery rehab/DME needs, and post-surgery check ins.  Will work closely with surgeons to assist with inputting surgery case orders.  Collaboration with external parties such as hospital partners and contractors.  Minimum Education, Training, and Experience Required: Graduate of an accredited Medical Assisting program with current CMA certification; OR Graduate of an approved practical nursing program with current Kentucky LPN Licensure Orthopedic background or experience preferred. Basic computer skills, specifically with navigating within databases containing highly confidential information. Basic Life Support and other advanced training as appropriate.
